F-HBCM - Takeoff with an erroneous centre of gravity (October 2025)

The BEA (French investigation board) published the enclosed leaflet.
During flight preparation, the ground staff incorrectly corrected the DOI in the software. The takeoff occurred with an erroneous centre of gravity.
OH-ATM - Passenger door handrail collapse (October 2025)
Event date: 12 November 2024
During disembarkation, a passenger grabbed the right-hand handrail. As he began to move down the steps, the handrail collapsed unexpectedly. The passenger lost balance, landed on the apron right flank first and sustained head injury.
RP-C7284 - Runway Excursion (October 2025)

On or about 1930H local time (1130 UTC) of June 06, 2024, an ATR 72-212A (72-600) aircraft, registered as RP-C7284, experienced a runway side excursion incident after landing on runway 12 of Butuan Airport in Bancasi, Butuan City, Agusan del Norte, Philippines. The aircraft came from Mactan-Cebu International Airport for a scheduled commercial flight to Butuan. The investigation determined that the probable cause of the serious incident was the flight crew’s failure to manage the aircraft’s directional control after bounced landing.
RP-C7283 - Runway Excursion (October 2025)

The aircraft had a runway side excursion incident at Runway 24 in Ninoy Aquino International Airport, Philippines. A Visual Meteorological Condition (VMC) prevailed at the time of the incident. The cause of the occurrence was attributed to the pilot’s decision to continue to land after a bounced landing on the runway, that resulted in a runway side excursion.
PR-MPN - Landing with Landing gear up (June 2025)
Event date: 15 June 2019
Shortly after takeoff, an electrical failure occurred, prompting the aircraft to return for landing. The aircraft landed with all three landing gear legs retracted.
G-LMSA - Engine fire (June 2025)
Event date: 23 July 2024
Whilst in the cruise, the aircraft suffered a contained failure of the No 1 engine and a subsequent fire inside its cowlings due to a leak from the fuel return line which had become sufficiently loose following the engine failure to result in fuel leakage. The flight crew correctly followed the checklist procedures to shut down the engine, and the fire was extinguished. The aircraft landed without further incident.
EC-MVI - Landing with Parking Brake ON (April 2025)

During the landing roll-out on runway 03L at the destination airport, the crew noticed a slight deviation to the left side of the runway, which was immediately corrected. After realigning the aircraft, the crew noticed it was slowing down more quickly than expected, and it came to a stop shortly afterwards in the centre of the runway. An inspection of the aircraft while still on the runway revealed perforation and abrasion damages to the tyres and rims of the main gear.
G-CMJM - Right Wheel NLG detached (March 2025)
Event date: 31 October 2023
Whilst taking off the right wheel from the nose leg gear detached from the aircraft. The detachment was not observed by the airfield operations department and the flight crew were unaware of the loss of the wheel. They continued the flight and landed without any abnormal indications or adverse aircraft performance.
LY-JUP - GO AROUND (February 2025)
Event date: 12 August 2024
On approach to Runway 27 at Guernsey Airport, the crew continued to descend below the approach ban altitude despite the reported Runway Visual Range (RVR) being below that required. As the aircraft passed the decision altitude for the approach, there was confusion and miscommunication between the crew which resulted in the aircraft remaining more or less level with the gear down.
G-NPTF - Stiffness rudder (Nov 2024)

On final approach, the pilot in command lost sight of the runway because of a sudden increase in the intensity of rain and strong gusty winds. He performed a go-around, during which the aircraft contacted the runway surface. The crew was unable to retract the landing gear after going around.

After the aircraft reached FL130, the master caution light and the engine electronic control (EEC) fault indication lights illuminated. Whilst engaged in fault-finding procedure, the ENG fire warning light illuminated. The crew complied with the in-flight engine fire checklist recommendation and shut down the engine.

The flight crew received a call from the Senior Cabin Crew Member (SCCM) who stated that there was a “faint haze or smoke” in the cabin. Shortly afterwards the SCCM reported that its intensity was increasing.
